Below is our Credit Application, a small Powerpoint presentation, a hinge location sheet, installation instructions for hollow metal doors and frames, and some other forms you may find useful.

To view the PowerPoint file, you will need the free viewer if you do not have PowerPoint installed on your computer. You can get the free viewer from Microsoft here.


  • Smith and DeShields Credit Application - If you do not yet have an account with us and would like to open one.
  • Credit Card Authorization Form - If you do not wish to open an account but would rather pay by credit card.
  • Hinge Location Sheet - Shows several different manufacturers hinge and lock locations from the frame top to the top of each hinge (unlike the standard forms showing the location to the centerline of the hinge).
  • SDI 122 - Installation and Troubleshooting Guide for Standard Steel Doors and Frames.
  • Flyer - View a 1 page document regarding what we can supply to you.
  • Power Point Presentation - This is very brief but gives you an overview of what we can do for you, how you can benefit from us, and some other good information. It is 6 clips and you can advance manually through these clips by clicking anywhere on the page or allow them to advance automatically.


The 4 forms below can be used when you have an existing door and frame and you are replacing either just the door or just the frame. Please print the form that is applicable to you, fill it out, and send it to us so we can be sure the hinge and lock locations will match what you already have existing.

If you are replacing a commercial door and the hardware with it but you are keeping the existing door frame, please use this form: New Commercial Door and Hardware Order Form.
